Meet Dr. Jaczko

I began my career opening a solo family medicine practice in Carlton, Oregon where I could connect with each patient on a personal level. My goal has always been to build a relationship based on trust, honesty, and communication. I have enjoyed working with each patient to identify their goals, explain procedures, advocate for their healthcare, and answer any questions or concerns they may have. 

I have worked in multiple venues over the years from urgent care, wound care, as a team physician for college and high school, and most recently with large regional medical groups. These experiences have guided my decision to move back to a primary and preventative care model that works directly with patients to educate and empower them in their journey for a healthier lifestyle.

My approach to medicine is to build partnerships. I look forward to being your physician with a patient-centric approach. I value humor, integrity, and will always provide your care with an attentive ear, open communication, and transparency.


I am originally from  New Jersey, but fell in love with the beautiful Pacific Northwest while in residency and have lived here and raised a family for almost 30 years.
